Failure Mode & Effects Analysis

PI Asset Integrity Ltd. have the ability to facilitate and complete Failure Mode and Effects Analysis (FMEA). The FMEA method examines potential failures in products or processes. It can be used to evaluate risk management priorities for mitigating known threats. The process identifies remedial actions that reduce cumulative impacts of life-cycle consequences (risks) from a systems failure (fault). By reviewing the parts of a system, and identifying the consequences of failure, the consequences can then be evaluated by three criteria and associated risk indices:
Following this process the overall risk of each failure, the Risk Priority Number (RPN) can be calculated, The RPN is used to prioritize all potential failures to decide upon actions to be taken to reduce the identified risk, usually by reducing likelihood of occurrence and improving controls for detecting the failure.
Completion of this process gives visibility, to our clients, of any potential exposure to risk, and gives the opportunity to formally record the mitigating actions taken to achieve any potential reduction in this risk. We recognise the experience and familiariarity that client operations and maintenance personnel can bring with their knowledge and history of being involved with the specific equipment being analysed, hence our approach utilises their input in the methodology wherever possible. If however this opportunity does not exist, the PI Asset Integrity Ltd. team has the ability to provide sound analysis using the data available to us working independently utilising our experience gained over may years in the maintenance and operation of a vast range of electrical, instrument and mechanical equipment . |
