Determining the failure path and true, root causes of failure, whether caused by maintenance, materials, human error or environmental conditions. At PI Asset Integrity Ltd we undertake a thorough evaluation of your systems and equipment failures by a sharply defined methodology based on the following:

	Data Collection
Investigation and collection of data from your systems allowing a full and specific understanding of your problems. Without gathering complete information to understand the event or events, eventual root causes can be undetected.
	Charting Identified Material
We use various methods of casual chcarting during which the various factors identified within the data collection can be recorded and identified to their specific fault groupings.
	Equipment Failure / Root Cause Analysis
Once all causal factors have been recorded and identified, the identification of causes are determined and reasons behind the events known, steps are taken to address the possible reoccurrence of these events.
	Recommendation Generation and Implementation
Finally recommendations for improvement or prevention of any reoccurrence are generated for clients. Who can then undertake implementation of these identified issues which will be suitable to their companies timescales.
